My Health On-Line

What is My Health Online?

My Health Online is a new, online service that Robert Street Surgery is now using in conjunction with NHS Wales.

DUE TO THE CORONAVIRUS PANDEMIC, ALL ONLINE BOOKING OF APPOINTMENTS HAS BEEN SUSPENDED UNTIL FURTHER NOTICE.
WE APOLOGISE FOR ANY INCONVENIENCE THIS MAY CAUSE.

To open an account you will need to bring to the surgery a passport, driving licence or birth certificate to confirm your name.  You will also need to bring either a bank statement or utility bill to confirm your address. Once you’ve created an account at home you will be able to use the features your GP practice has made available, which may include:

  • Book and cancel appointments
  • Request repeat prescriptions
  • Update your personal information
  • View your medical record summary

How Do I Register for MHOL?

Before you can use all of the facilities found on My Health Online you should register to use the service.

In order to register you must obtain a registration letter from your GP practice, to do so please complete the proof of identity form which is downloadable above.

You will need to bring this to the surgery with proof of address.  If you do not do this you will not be able to use some of the features.  Once you have completed the required forms your GP will supply you with a registration letter in order to access MHOL.

Once you have received your registration letter go to the My Health On Line registration page.

Enter the Practice ID which can be found on your registration letter

Enter the Account ID and the Linkage Key from your registration letter.  Then complete the account details as prompted.  An email will be sent as verification of your completed registration, so ensure that you use your own personal email account when registering.
